Muchas gracias a todo el Staff del Centro. Mi madre estuvo un mes y se sintió muy cuidada. El lugar muy limpio, muy tranquilo y los terapeutas siempre atentos. La comida muy rica manifestó mi madre. Gracias a todos ?♥️ muy recomendado. Nos han hecho directamente el trámite con todo lo que necesitará mi madre en casa.

The facility is very old and not very maintained. I give this facility one star because there was truly gem in there, Jackie. If it wasn’t for her our stay would’ve been horrible. She does everything from the heart and is very caring and helpful. Unfortunately, she is the only person that cares there!!
Everyone is extremely rude & Not willing to help. My dad would have his light on for service for hours before somebody would come in. I would have to call the facility many times to ask him to help him. I would not recommend this facility at all.
The food is horrible as well. I wish I would’ve read the other reviews prior to having my dad in this rehab.

My sister went here on 9/25. On October 7 she went into septic shock due to severe infection of a bed sore which obviously was not treated properly. She was sent to ER and died on October 9th! The doctors at the ER were shocked and appalled at the size and severity of her wound! 45 cms/17 inches! Went from mid back around her hip!! Deeply infected and tunneling into her bone! In 14 days it went from 4 cm x 4.5 cm to 45! I am not exaggerating! Dead within 14 days! Do not take your loved one here!!

Thank you to the physical therapists that worked with my mom(Mary) when she was at the facility in May 2024. Aileen and Shri were the best. She hasn't stopped talking about you since she came home on 5/29/24. You were able to help her get some her mobility back. The only issue she had was with her food. She was on a heart healthy diet and they didn't get her the proper food until she was about to leave. The nurses and attendants were always there when she needed something. She truly appreciates the time you all spent with her and the conversations you had with her.

My brother spent 16 days here. They almost killed him by administering fentynal to him when he has never been on it. He ended up in ICU...worst call to get especially when you find out why. He was also not being rotated and would sit for a while before being changed. Right before being admitted to the hospital I went 2xs to discuss his care. He called me the night before and said "get me out of here they are going to kill me". If you love your family member you may want to think twice. Me and my family were there almost everyday and this still happened to my brother.

I would like to thank Fair Oaks Health & Rehabilitation for their care of my mom. I would like to specifically thank Rhonda Lewis.
My mom was previously in another rehab center and the level of care and attention she got at Fair Oaks was just incredible.

Specifically, Fair Oaks Health & Rehabilitation did the following very well:
• Fair Oaks is well kept. The facility is clean and the staff is attentive and friendly
• When my mom pushed the call button, the aids and nurse came right away
• The nurse bathed and attended to my mom. One of the aids, braided my mom’s hair is such a beautiful and stylish manner
• My mom is a strict vegetarian. The chef came to my mom and asked if the entrees suited her needs
• The Kaiser doctors would also seek me out and speak to me regarding my mom’s case. They were very attentive and went through potential diagnosis and recommendations with me

What impressed me the most at Fair Oaks Health & Rehabilitation was Rhonda Lewis.

Rhonda Lewis called me and my brother every day updating us. She would visit with my parents every day, sometimes multiple times daily, confirming that my mom was comfortable. My mom was at Fair Oaks for about a month. Towards the last two weeks of her care, Rhonda setup daily meetings with nurses, aids, doctors, my dad and mom deciding on mom’s next steps. Rhonda also had my dad meet with Sunrise and Care with Love. Rhonda also meet with these two outside parties with my parents. My parents decided on private nursing care and after my parents left Fair Oaks Rhonda continued to follow up daily with my parents, my brother and I.

I am very impressed Rhonda Lewis. Her attention to detail and care is just incredible. She has done more for my parents than any other person so far.

Kudos and Thank You to the Rehab Therapists (Physical and
Occupational Departments). I would like to thank this Department for exceptional services. Under the guidance and direction given by Chetna, this team represents one that is united, purpose driven and results oriented. I worked with Shri, Aileen, Mike and Djahida. Each person brings their own style. ,They each show patience and empathy. With them, I felt comfortable They paid specific detail regarding progress safety.

Horrible place where very few people cared. Some staff refused to put on a mask although I was in isolation- others got my meds wrong and weren't responsive to my physical needs. I was neglected and ultimately my family removed me for my own safety. The uncaring staff that work there and would fall asleep behind the desk should be ashamed of themselves.
