My grandmother recently stayed at San Gabriel for rehabilitation after her hip replacement, and the care she received was outstanding. The team of therapists and nursing staff was diligent, attentive, and genuinely cared for her well-being. A special shout-out to Debbie NP, who was incredibly supportive. She addressed all of my concerns with patience and compassion, making this difficult process easier for my family and me. I highly recommend San Gabriel for anyone in need of rehabilitation or medical care for their loved ones.

The staff at San Gabriel Rehab are so incredibly kind and compassionate. I cannot say enough about how loving they have been as we transitioned Nina into her new home. From the directors of nursing to the therapy staff from the aids to the activities Director and so many other people, everyone has been so warm, welcoming, and encouraging. Long overdue review, (but hey, I finally am getting this knocked off my to-do list, hah). No one tends to think about long term care, at least until their loved one seems to suddenly require it and they're rushed into making a decision. Such was the case for my mother. After providing in-home care for my mom with moderate-severe Alz, she had a fall related to an infection, which also caused fairly significant delirium. After a hospital stay to clear up the infection, we chose San Gabriel for her rehab, (which was covered via Medicare through her advantage plan), and they arranged transport for her. She spent several weeks, and while there she got the benefit of several different therapists, (physical, occupational, speech, etc), which was a big unexpected perk in my eyes. They even thought she may have some vision impairments unrelated to her dementia and arranged an optometrist to see her, (which revealed pretty advanced AMD). The staff seemed very caring and by the time she was cleared to return home we were happy to see her get some well-wishes from her caretakers.

Fast-forward several months, and I was being sent out of state for a work function for just over a week, with my spouse invited to attend on their dime as well. Never having used respite care before, I was hesitant but reached out to San Gabriel once again. Their intake staff and coordinators were great at 'holding my hand', and explained how everything would work on a voluntary self-pay basis. Their financial arrangement was more feasible than I had expected, and we started the process. During our week away, we managed to get updates as needed to help be reassured she was in good hands again, and even with her Alz, she remembered some of the activities they had her participate in. (She managed to tell us she went to church services, as they have volunteer pastors come by, which was great to learn about for her, and she also said something about some type of arts and crafts, though I myself can't recall what she made now....oops). This stay allowed us to take a week for ourselves, which as anyone who is a primary caregiver for a family member knows is something vital for our own health, though we never seem to actually do that for ourselves.

All said and done, (after this long review, but felt it important given you're not buying a car, you're looking or needing care for a loved-one), I would happily recommend San Gabriel. We had come close to investigating her being a full-time resident thanks to our two experiences with them, but by that time, mom had started declining fairly quickly, and we went the home-hospice route instead for her final time. If we hadn't been in a position to do that, (mentally, emotionally, financially, etc), I'm pretty sure we would have spoken to them about all our options including what they may have been able to offer her. None of us want to think about LTC, (or even short term rehab scenarios), but if you find yourself needing some help, it never hurts to call and speak to them, set up a meeting or a tour, etc. Hope my review helps anyone in a similar position as we once were.

We couldn't be more pleased with the care my husband receives at San Gabriel Rehabilitation and Care Center. After complicated infections which required ICU and rehab hospital stays, he lost almost all of his strength and he lost all mobility. We were warned by the admin who gave us a tour that San Gabriel was a lower level physical therapy than the rehab hospital he had just left. As it turns out, he receives more occupational therapy and physical therapy every day at San Gabriel than he ever received at the rehab hospital. He gets some form of therapy every day, including weekends. As another reviewer said, you get out of it what you put into it, and if the therapy gym is open, you can go work out. The therapists seem to be as skilled and talented as any we've worked with.

The care givers (medical and technical) are very competent and for the most part are very kind and compassionate. Of course, there are a couple who are less helpful, but all things considered, the care my husband is getting is as good as any he's received anywhere.

As we were making the choice between Skilled Nursing Facilities, we could not have been more pleased with the administration. Aaron had my husband's transport for medical procedures arranged before he even got moved there. We needed to get moved fairly quickly and San Gabriel made that so easy and fast. He was moved within 12 hours.

Even the food service is satisfactory. There is a good variety of meals offered and they're actually usually pretty good. No complaints at all in that area.

We were also impressed with the administration following up with us after he was settled in. They were genuinely interested in how he was doing and the couple of small things we mentioned were addressed so quickly we were pleasantly surprised.

We are very pleased with San Gabriel Rehabilitation. My husband has a pretty long road ahead, but he feels very positive and really supported at San Gabriel.

It is a good place to work at. The department head care about the staff. We have team work here. I started working here in 2012 until 2019.
At that time I had a death in my family and the department heads had call me to see what they can do. Then I left here but I was not happy. I came back in 2023. I realize this place is where I am suppose to be.

These 4 stars are for the 4 medication aids that l was in contact with in my stay at this place. You know who you are❤️ They were professional and listened to you about your pain when they came. The ones that suppose to make beds and give you water, l will not waste my time to comment my battery will run out it’s so sad y’all make it look bad for the ones who like their job.God sees all you do. You will have one of those beds one day. KARMA!!! As for my little incident with a patient l charge it to the caregivers.
I thank God l’m in my right mind and can live move and have my being.
I am praying for every patient in that place that’s being neglected and mistreated in the name of JESUS . PSALMS 91:1-10. God is all knowing, and is present at all times.
I didn’t check the reviews before choosing this place and l do believe the name of this place should be changed.

Don't allow your loved ones to be placed here, I wish we didn't. He is declining a lot faster, and he is there for rehab and to work on his walk. He now can't hardly move and is a extreme fall risk. One of the nurses (Idk her name, but I know her face) said word for word- HE HAS THE RIGHT TO FALL”. EXCUSE ME? That is NOT OKAY! He is a fall risk, it is their duty to do what they need to, to not let him fall. He has fallen 4-6 times that we know of, and he has bruises up his arms and hands. Also, my grandpa said he fell and hit his face, (He has a big red mark, I took a picture) but when I ask around the nurses, they tell me they don't know if he has or hasn't. None of the staff can ever give me a direct answer.
Also, every time I go see him, it smells like urine, and when I visit, I never see a staff member till I walk out of the room and go find one myself. when you press the call button, they don't ever show up till after 30/35 minutes. At least every time I am there. I get busy, but when I am there, I always observe and see staff hanging out behind the desk or talking to each other and they seem like they just don't care. At least run over there to make sure he hasn't fallen out of bed or is injured! They need to have alerts in the room or on the bed informing them he isn't on the bed or is trying to get up, like hospital beds. I was there yesterday to visit, when he didn't have water near him, it was at the opposite side of the room of him and the lid was covered in what looked to be old food. I asked to get him a new cup and fresh ice water and they told me to do it myself. SO, I DID. I am frustrated and hate seeing my grandpa in this position. If you are going to run a facility like this place, MAKE sure you hire good caretakers who care about people.

On 05/20/2025, at about 1515 I went to visit my grandpa again when a (B/F) with short hair wearing light blue scrubs asked me what I needed when I walked out to the hallway while the call button was on. I stated that my grandpa needs to use the bathroom when she rolled her eyes towards me and said, “He just went not too long ago”. I stated well he is elderly and can not control his bladder and everyone is different. I also stated that he can use the bathroom anytime he needs to go. Like a HUMAN BEING CAN!!! She then did not come help. About 5 minutes after another nurse came in and asked what we needed. I also showed her that my grandpa has lost strength in his hands and cannot press the call button. She informed me that she would come in and check on him again in about 10/15 minutes. SHE DID NOT. Nearly an hour goes by before she comes back. His water was not close to him, and I tried to watch him eat. He can’t hold his silverware and they did not cut his food up like they said they do. Also, while I was visiting grandpa that day, He stated he needed to go to the bathroom again, so I asked the male nurse and he got him to bathroom and grandpa said he’s done. Well, we didn’t hear or see anything in the toilet. The nurse kept asking him if he went, he stated yes.

On 05/21/2025, We received a phone call about them rushing my grandpa into the hospital due to blood in his urine. Just got results after being at hospital and they say he has no fluid in his bladder and that he is severely dehydrated!! They said it looks like he hasn’t been fed or given any water in weeks. Which he has been at this facility for 3 weeks. I have witnessed with my own eyes that the staff here do not take the time to help him or make sure he is okay. He told me at the hospital he is hurting. He has dementia!! He is not okay and cannot take care of himself. Explains why he couldn’t pee yesterday and he thought he had. I am a frustrated and upset grand-daughter and I do not like to see my grandpa mistreated or anyone for that matter.

ALSO!!!! THEY TOLD US THEY WOULD PAY THE BILL OR NOT GIVE US THE BILL BECAUSE THEY ARE TRYING TO HIDE THE FACT THEY NEARLY KILLED MY GRANDPA!! NOT OKAY.. ER DOCTOR INFORMED US THAT WE GOT HIM OUT OF THIS PLACE JUST IN TIME.. OR HE WOULD BE DEAD!

My father was there, and nursing / dietary and housekeeping were awesome the best I ever experienced, therapy was the top as well. I would highly recommend San Gabriel.
