This place was an absolute nightmare. My mother spent 3 weeks here. The nurses are the rudest people I've ever met. Much of the staff is absolutely incompetent. The social workers were inept. When I did speak to anyone I only ever received incomplete or inaccurate information about my mothers welfare.
She was transferred in the middle of the night, and as she's in her 80's she was very confused, she began calling her family (my mother has no memory issues). As we began calling the facility it took an hour to get a real human to pick up. The person told me the place was "to big" to know where she was. She then proceeded to put the phone in her pocket where for 30 minutes I could hear her walking around talking with other people and laughing and not once asking where my mother could be. It wasn't until my sister called and threatened to call the police did anyone actually find my mother and told us she was ok and in a room.
No one ever returned my many many phone messages, I was in 1 meeting to get an update on my mother, and the information I received was incorrect, I then sent an email to validate the information and the answer I got back was ONE badly formed sentence that had pretty much nothing to do with any of my questions. On 1 occasion I receive a "proactive" call about my mother's health coverage, they asked that I change my mother's coverage it to give her more time at the rehab. (I did not do that)
The floors were dirty, the dressers were dirty and broken and my sisters witnessed bugs on more than one occasion.
I called 3 days prior to arriving and asked for her to be released. I flew in from NJ to get her and I was met by a surprised social worker who claimed she never got the orders. On the DAY OF RELEASE, I went to pick her up and the nurse told us she didn't have the release papers and we couldn't take her home. I informed them that we were leaving and that I would wait in the waiting room. A few minutes later another supervisor showed up to give me the release papers she told me they were on the FLOOR and that's why the nurse didn't see them.
After I took her home, I waited 4 days to call Luxe to make sure that she was going to get PT at home (as she is covered for that) and they told me they didn't know why the service had not called me yet. I then called the at home PT service, and they told me they never received the order from Luxe, so they took it upon themselves to call Luxe and got it resolved for me.
Since my mother's departure, she has gained weight, is more her old self, is able to walk and dress herself.
This place is a disaster, not enough staff and just not friendly staff.

My father checked in here last year. I felt there were several missed opportunities. For example, they knew he had a gait/balance problem due to movement disorder, but eating his first 12 hours of being there he fell and hurt himself. Additionally, the staff would sometimes bring his food into the room but leave it out of his reach until it got very cold. I acknowledge that the staff was friendly and helpful. It seemed to me that the problem stemmed from the board and upper management, because the nursing and worker staff seemed overwhelmed with too many patients to care for. Also, turnover was high resulting in not enough staff to provide adequate care. We got my dad out of there in 48 hours. Anyone who wants to check there loved one in, I advise you first take a tour and ask A LOT of questions.

It was easy to check in for a visit. I asked the nurse for a little thing for my mom and she did it immediately. Everything was. Really clean and everyone was very nice and helpful. Moms 2nd day there and she looked good and sounded good. ❤️?

Had a very scary and bad experience at this place. My father had a stroke and was sent to this place and while he was there he had a bad fall and no one said anything . He was starting to decline rapidly and wasn’t able to eat or talk anymore. They took some test and saw something was off with his kidneys and just ordered medicine that was suppose to take a couple days to arrive . I was standing there watching my father slip away from me and kept asking the head nurse if we can send him to emergency room and she just kept saying he’s fine he doesn’t have a fever . It had gotten so bad that my mother had to call the police so they can send him to a hospital and it turned out he had a brain bleed from the fall and his kidneys were failing . If my mother had not been there that day he would not be here with us today . The fact that they saw something wrong and did not want to take any action is very troubling. If you have a family member I would recommend you not letting them go there .

Horrible treatment of my father. He was mobile when he entered the facility, but bedridden by the time he left. They didn't do rehab as much as they should have, and they put him on stool softeners that he did not need. In addition, we're pretty sure that he got c diff while there. He's now in ICU with near kidney failure due to severe dehydration from constant diarrhea.

Stay away!!!!!! They are understaffed and treat people like animals. The level of care is shameful and the food is not fit for human beings. I wished someone would have left the kinda review that I am leaving I would have never come here. This place has no shame. Should be investigated by pasco county for
Inhumane treatment of patients. The The food is the lowest quality food they can buy. It’s all carbohydrates. It’s all salt. They serve better food at the prison. It’s greed driven bottom line profits make a fortune from the patients from the insurance companies, but they treat people like dirt. They should be ashamed of themselves. The most unhealthy meals for people that are requiring healthy food to regain their strength instead the greedy owners only care about profits and treat their patients like animals. I honestly don’t know how the corporation that owns the lutz facility sleeps at night disgusting. My father was sent with diet requirements from edvent health stating no salt I saw his meal ticket on his tray and it said regular food his meal some kinda creamed thicken served on biscuits that was so salty I took it away from him he just had stroke.
