Meeting a friend at cafe noir I ended up making the mistake of assuming this parking lot was for the coffee shop as it is the only visible business on the street. The sign my eyes went to while pulling in was the highest up which read customers only but didn't state the name of the business. Upon parking there were no additional signs on the stalls or fences and there were many open spaces so I was not causing an issue for the business. After a quick stop at the coffee shop I returned to find my car had been immobilized with a boot and a giant sticker was stuck to my window which left tons of sticker residue when removed. I was detained there for over an hour and had to pay 75 dollars to have the boot that was placed on my car by clinic employees removed. Anyone could make this honest mistake as the clinic is not at all noticeable from the south east direction of the lot. If your business is hidden in the back corner of the lot and there is a much closer coffee shop with no parking of it's own EVERY sign should have the business name posted and there should be abundant signage and/or business logos within the lot. This seems like a trap/scam the clinic owner is using to take money from anyone who doesn't thoroughly read every single sign while driving into the lot. Especially if you are approaching westbound on 200 S and can't even see the clinic.

I was recently discharged as a patient at City Creek. During my two week stay, I was given the wrong medications, slapped by a male RN, dumped out of a wheelchair onto the pavement. Kool-aid is served with every meal. Food is boring and small portions. One dinner was watermelon and bread. Many staff are rude, including the facility supervisor. Can't recommend this place to nobody.

Thank you from the bottom of my heart. At first I had my doubts but as time went on I got to see firsthand the care and compassion the staff portrayed. There are too many names to have to name them all that is what a wonderful job they all did but there is one name I would like to give a great big thanks to, Katie the caseworker. I highly recommend this facility for your physical therapy needs.

City Creek Post Acute is a lovely facility. All of the faculty here truly, and wholehearted come in and try their very best for every patient they have. The admissions team works hard to make sure that any accepted patient is greeted by friendly staff, that the rooms are clean/ready for new residents, and that they are able to get transportation to the City Creek Facility. The social worker goes above and beyond, not only helping residents apply for various medical benefits through the government, but truly cares about each patient's mental wellbeing. The nurses and CNA's are constantly working to make sure every patient gets the most accurate and efficient care they possibly can. Patients who suffer from memory loss conditions are also allowed to freely roam if they wish, but City Creek goes the extra mile to make sure these patients have trackers that prevent them from leaving the facility and getting lost in the city. This does not mean that residents experiencing memory loss cannot leave the building though. I have witnessed nurses and CNA's take these patients on walks throughout the beautiful neighborhood where City Creek is located. But my favorite part about this facility is the sense of community it provides for its residents. This is a place where patients not only get the treatment/help that they need, but they can make friends and participate in fun activities. The head chef here prides himself on the food he makes, and always ensures that there are substitute meals incase patients do not enjoy the main meal and would like something else. This facility is full of LOVE! And to me, that is what is most important. If you are looking for a safe, loving and fun environment to send your loved one to, this is the place! The staff at City Creek is just waiting with open arms.

Subject: Serious Concerns Regarding Mismanagement at City Creek Post Acute

I am writing to share a very serious incident at City Creek Post Acute regarding my mother’s care. She was scheduled for surgery on May 21, 2024. However, the day before, the head nurse, Deb, informed us that the surgery had been rescheduled to June 6.

On the morning of May 21, my mother received a call from the hospital asking why she wasn't there for her surgery. It turned out the surgery was still scheduled for that day, contradicting what Deb told us. This error had severe implications:

1. Disruption to Medical Staff: The operating room staff and surgical team were prepared for a surgery that didn’t happen.
2. Increased Patient Suffering: My mother, in significant pain from a shoulder fracture, had to endure more pain because the surgery was unnecessarily delayed.
3. Further Delay: The earliest new date for surgery was a week later, prolonging her suffering and recovery.

When I spoke with Deb, she seemed unconcerned, blaming a mix-up with another patient and her scheduler. This lack of accountability and empathy is unacceptable in a facility supposed to help individuals recovering from surgery.

This incident has caused considerable distress and raises serious concerns about the communication and patient care standards at City Creek Post Acute. I hope this review prompts an investigation and necessary changes to prevent future issues.

This experience has been incredibly frustrating and disheartening. I cannot recommend City Creek Post Acute based on this experience.

Talk about a criminal place released my father homeless on a Sunday with no where to go they should be brought up on charges luckily we got him to me but if he would not of got a hold of me he would most likely be dead. If you have someone here you care about at all move them somewhere else before they do some of the horrible stuff my dad has been sitting here telling me.

Very caring admin team and great therapy. I had an opportunity to observe one of the activities with recreational therapy and physical, occupational and speech therapy. I loved seeing the residents smiles on their faces and the meaningful interaction from patient to patient and staff to patient.
