This is by far the worst place I've seen, and I would not recommend it. For one, there's blood stains on the floor. How is that not a hazard? Secondly, as soon as you walk in, there's an intense urine smell. There is no proper ventilation to ensure the residents or the public isn't inhaling such fumes. Third, the food is not edible. Frozen chicken patties and cake that look as if someone took a bite out of it. Plus they serve food that goes against dietary/religious restrictions. I don't know how the workers work in such a disgusting environment or how they care so little for the residents. We literally had to ask for toiletries and pillows. How is that not common sense to provide those things for newer residents? This place is ghetto point blank, and I will forever blame the hospital coordinator for putting my loved one in such a gross environment.

Warm and loving facility. Every one we met was very cheerful, respectful, hospitable and welcoming. The director took us around for a tour and we were really blessed for allowing us to volunteer to be a part of their family. Thank you Ms. Angela for this visit. God bless you.

Everyone is great and very helpful. They are very helpful from the time I check in at the door and to the floor my husband is on. They take great care of my husband also. I would like to thank everyone for taking great care of him for me. Thank you again and stay safe.

My mom made the horrible mistake of choosing this facility because it was close to home. First I will say that some of the staff is very loving and kind also there is a supervisor Tanya who was quite helpful and as sweet as pie. However it takes more than just a few good people! The lack of care is completely unbelievable. My mom’s roommate had to change my mom’s diaper because no one came to help. The place is filthy. The toilet in my mom’s room was never cleaned and stayed the way you see it in the photo even today it still hasn’t been cleaned. They need proper training in many areas. If you truly love your family members NEVER send any of them to autumn Lakes. You will absolutely regret it.

The food sucks, when you put the call light on no one response for 10 minutes. Patients are all given water to take meds from a single pitcher of water in a small cup. The person I was visiting was given hot water only in place of coffee for dinner. It was not reported. One Patient screams morning, noon and night very disturbing for rehabilitation. The staff has been very accommodating otherwise, some not so nice.

My mom has been here 3 weeks and they only give patients a bath 2 times a week they claim the water was broken one week needless to say I had to give her bath was the only way she would get one and they stole my moms thermal cup. They don’t tend to them if they need to be fed your loved may just starve if you can’t visit daily.

Great place! We give it two thumbs up. The doctors and nurses and therapist and all of the staff that have been taking care of my brother are all wonderful. I wish that I have known about this place 7 years ago when my mom was living. We had an awful experience when she was in a rehabilitation center. She was hurt and it was covered up. So we were very worried about my brother. So we really Thank God that he showed me the way here at Autumn Lake. Thank You to all the Men and Women for taking great care of not just my brother but all the patients there at the center. God Bless each and every one of you for caring for others and having a huge heart and putting the patients first.

Staff was extremely nice. Facility was slightly dated (so minus 1 star) but it is clean and no foul orders. All of the patients seemed to be very nice also which says a lot.
