Recently put my mother in this facility to have her closer to me. After a few communication issues and meeting with the executive director, Jack, I am happy with the resolution and our decision to relocate Mom to this facility. It is a smaller facility than the last one and the staff is more attentive to her needs.

Our late dad's friend is at this facility, Green Cove Springs Rehabilitation and Care Center, and he has no family to care for him. When we checked on him this week and saw the facility and the conditions he was living in, we were appalled. No human being should be in this place. There is an utter disregard for any type of even basic care. The rooms are beyond disgusting, dirty, filthy, piled up with junk, unsanitary, garbage lying around, an absolute breeding ground for bacteria and viruses. They obviously do not want to pay a high electric bill so they keep all the rooms sweltering hot, thus spreading the growth of more bacteria. I do not understand how this place has any type of license or accreditation. They must know in advance when someone is coming to inspect it and they must get everything in order for just that visit. I am not exaggerating on anything that I am sharing. I'm still in disbelief that this is considered a long-term care health care facility. No humans should be subjected to these living conditions. What's really scary is that medical professionals at the local hospital and professionals at the local fire & rescue "know" this place and everyone encouraged us to get our friend out of there as quickly as possible. RUN FROM THIS PLACE. DO NOT LEAVE A LOVED ONE HERE. Even basic, basic care is withheld and completely non-existent.

What an amazing transformation this facility/home has made in the past few years. The staff members are caring. Everyone knows the patients by name but more important the patients know the staff by name. I've visited my aging aunt at least weekly and have only good things to say. I see Love and warmth and it really has a family feel. I am so pleased with the care. The doctors are approachable and will keep you informed. The building is not new and could use some updating for sure but the care inside is what counts. Thank you again for all of your hard work and for taking such good care of my aunt.

Avoid them at all costs! Firstly, my father does not receive appropriate care: He has a roommate who has COVID and neither was wearing a mask when I recently visited. The room was sweltering hot with no toilet paper and NO HANDSOAP. His nails were dirty, his beard covers his mouth so he has trouble eating, he is unkempt. Physical therapy is non existent so my dad just lays in bed. I have repeatedly asked for my father's doctor to call me (as I live 14 hours away) to advise me of his prognosis and update me about his long-term care but that message is either never given or ignored. I paid $110.50 for my father's medical records to give to the VA but they have not mailed them to me yet. I requested them 7/18 and I've called 8/1,8/2,8/3,8/5 and again today with no response.. When I call to follow up, the records department hangs up on me or sends me to voicemail. The one time the rep answered, she "faxed" a copy but the copy was sent piecemeal (a few pages at a time) and illegible so it is not doing me any good. The only person who calls me regularly is the business office rep who lets me know the cost for private pay is $318/day and when it's due. That's over $9500 a month for shoddy care. RUN, don't walk, away from this facility.

Update: a month later, my calls are STILL being ignored by the Records Clerk, so I do not have the records for which they charged me $110.50. This whole process is delaying my Father's VA benefits. I have emailed twice, called 9 times and no one is assisting. Unacceptable!

I have been at the facility for almost a year now. I have witnessed the nursing staff be respectful and caring to all of our residents. We do have challenged residents from time to time and I have seen the staff act in a professional manner and deal with inappropriate behaviors. Everyone is happy and pleasant most of the time. No facility is perfect, but we try everyday to be the best we can be and acheive the standards that our residents deserve.
We have Activities on the calendar to improve the residents Quality of life, like bingo, entertainment, spiritual functions, cookouts, parties and family nights. Social services is wonderful and willing to go above and beyond to help our residents get what they need. Our maintenance department is fantastic and willing to do whatever is needed. We have an awesome management team that work well together and a amazing therapy department. I am proud to be part of the team at the creek. If I could I would speak about each of our staff that go above and beyond each day.

Just want to give a big shout out to Social service director Dana Lucas for all the help and hard work she put in on getting my dad Jesse Thomas released to go home. I'm so thankful for everything she helped me accomplish to make the transaction smooth. My dad lived at this facility for over 3 years and we never had any problems. Also want to thank the staff for loving and taking care of my dad while I was unable.

Like the review of Sharon below I've had a horrible experience with this facility.
It has been terrible, gut-wrenching, stressful, and has been a lot of poor communication and non-professionalism and concern when it comes to understanding what people are going through when they have their loved ones in this facility doing the covid-19 era.

I made about 10 calls to this facility over a period of 3 weeks was very polite left my name and two phone numbers and I was never even contacted in reference to my family member that was in this nursing home. If there was a negative rating I would give it a negative 7!
Also do diligent research when deciding to place your loved one at this facility because a guick Google search will shows that Governor's Creek was involved in lawsuit in the millions regarding Medicare Medicaid fraud. We are hoping and believing that the staff and administration and everyone involved in that situation has been fired and replaced hopefully?

Please avoid this nursing home for your family. First, like a lot of places, you have to monitor them closely or they won’t properly care for your loved one. This was my family’s experience. In addition, they will discharge you with no directions, no guidance on how to continue care. Had to call back to find out how much food to give and when to medicate. They sent my dad home with enough liquid food for 2 days, and no order for more, which apparently takes two weeks to go thru. They Did not mention that some pills couldn’t be crushed, kind of important information that would have been nice to have received. There were one or two nurses who did care, and a therapist. However, the head nurse couldn’t care less about her patients and has a terrible, rude manner. Avoid this place if you have a loved one with health issues!

Also, see below where the owners as you to contact they to hear more about your issues. Well, we have tried to contact them repeatedly before the review, nothing changes.
