The only way this place got 5 stars is because they must be from friends and family of staff. This place left my mom in a wheelchair and she fell out and ended up in the hospital the whole reason for her being there in the first place. This place has old TV’s antiquated beds with railing, which they are not even supposed to use. Room was filthy and falling apart they would feed my mother and put sugary substance on her food to get her to eat. There was locked emergency doors. There was no sense of emergency for anything except maybe for workers to go on break people sitting around chitchatting while residents scream for help truly sad. This place would not lift a finger to help me move my mother out of this facility, while the nursing Director bragged about how the lot across the street was theirs and they were going to build a bigger facility. Meanwhile, they barely did anything for the people that were already in the smaller facility , truly sickening and sad. I will let the photos speak for themselves. I do not recommend any kind of long-term care from this place. The photo is of my loved ones wrist, which was swollen and red and took hours upon hours to get an x-ray done. They did nothing to help. If I wasn’t there only God knows what would have happened.

The appearance and presentation of the facility is not appealing. I understand that it is an old building, but a coat of paint, some great landscaping can do wonders. From the moment we entered the facility, there was a smell that was similar to a moldy, dampy, old smell. The person at the desk was not overly welcoming, but it was the  early hours of the morning so, ok, I give her grace.
The room where my loved one was placed looked old and outdated. The furniture was not in the best condition, specifically the bed, which creaked when changing position and there was signs of rusting on the railings. There were some items in the space that obviously belonged to another probably previous resident. The intake was haphazard. I am yet to get an explanation of the policies, routines, visitation, things to expect, do’s and don’t’s etc., from anyone. The physical therapist was the bright light the next morning, and did explain that a plan of care would be made for my mother and what was expected to occur from the rehab standpoint going forward. I appreciate her working with my mother, though it was for a short time. The “nurse” was not professional and did not exude knowledgeable, confidence, great clinical skills. That was not on display each time she and the nursing staff were observed. My mother communicated being yelled at and told, “stay in the bed woman”. The lack of nursing skills was confirmed when my mother had a medical emergency two days later and fire rescue had to be called. The nurse spent valuable time throwing water on my mother’s face to revive her instead of doing a comprehensive nursing assessment to check for circulation, airway and breathing. Thank goodness, a family member was present to alert the staff when my mother became unresponsive, otherwise goodness knows what the outcome would have been. My mother was taken to the hospital and remained there for 5 days, and is now being cared for at another SNF. During her hospitalization, not once did anyone check with family or call the hospital to see how my mother was doing. After all, she was at that time one of their residents, was she not? She was hospitalized for 5 days. We decided, we would not have my mother return to that facility, even if we had to take her home and care for her there. When the family returned to the facility to pick up my mother’s belongings, staff initially could not find them and another resident was seen using my mother’s property, specifically her bible, ( I do hope it was helpful to her, but really?) At this point I will not recommend this facility to anyone. I know there are a number of approvals that is needed from numerous agencies, such as AHCA, CMS, JACHO, etc. for a facility to remain open, frankly, I don’t understand how this facility could have passed their high standards. This is a very honest and heartfelt depiction of our experience at Aventura nursing and rehabilitation center, in only a short period of time. It is in no way done to defame this organization, but only to hopefully cause there to be a change, a time for reflection, a pause, an assessment, and a revision of how business as usual is carried out, and what the expectations are for the caregivers and how they are held accountable to carry out what should be, excellent care and service to these elders and what they deserve. Today it’s them, tomorrow it can be us. I would like to get feedback on this submission.

I believe the Nurse Assistant’s name is Ms CeeCee but she has been a pleasure to work with. I’ve asked her so many questions and she never, ever hesitates to accommodate me and my daughter. I thank the Lord for guiding us to Aventura Plaza Rehabilitation and Nursing Facility.

My mother (Rebeca Ruiz) is currently in this center and the care of the nurses, the assistants and the excellent rehabilitation team is wonderful, not to mention the cleanliness and the meals, everything is super well organized, the attention of the front desk, everything is incredible, highly, highly recommended.

Many nursing homes are being closed
And under funded. So we all need to
Be a little more understanding. But i
Do have one question where is all the
Money coming from for all the boats
In the area.

Thid is thr worst place ever! I don't know where did they got the nurses and help! They are rude and nasty with the patients! Will never bring anyone that I love to this place! I should report it to the Health Department .

I like that it has ample space to visit your loved ones, but it needs a major renovation. It would be great! if deodorizers could be placed on every single floor and on the elevator as well. I am extremely sorry, but the facility has an unpleasant smell to it.
