Our mother has been in Crescent Health & Rehabilitation Center for a month now. The entire staff made us feel comfortable from the initial consultation. The nurses were always responsive and considerate. We are very pleased with the experience the past month.

My mom received excellent care at the Crescent Center during a recent rehab visit. I can't say enough good things about Crescent. I want to thank them all, especially Jill. I would have been lost without her. I would recommend them to anyone in a heart beat. My family and I are very grateful to all the staff, doctors and nurses. I would like to thank personally Alexis, Joanna, Dante and Katherine the receptionist.

I have visited many rehab places in recent months in the Venice-Sarasota area. Crescent is the best. Thank you again.

Not long ago I already expressed my opinion about
Crescent Health & Rehabilitation Center. Our mother is there in nursing home. It was a very difficult decision, but we had no choice. And initially, it was very difficult for her, because she doesn't speak English. But a miracle happened, thanks to one of the employees of this institution. And the name of this employee is Aurora Dimitrova. We are grateful to all the employees of this institution for their good attitude towards our mother. But it turned out that Aurora can speak the same language, that our mother understands and that was salvation. Many thanks to the administration for having such workers and many thanks to Aurora for her hard work.

I would not leave my dog at Crescent Health and Rehabilitation Center! I recently had a friend who spent almost two weeks at this facility, where I visited her everyday, and observed shocking behavior on behalf of the staff and administration and unacceptable conditions.

When I learned my friend had been transferred to Crescent from Sarasota Memorial Hospital (SMH) in Venice (where she received excellent care following surgery), I tried calling the facility, but their phone was “out of order.” I thought this was surprising, but as it turns out, I could never reach this facility by phone, which was shocking. On subsequent days the phone wasn’t out of order, but no one answered the phone, and I could never leave a message!

When I drove to the facility, there was no one at the front desk, but I noticed the business manager was in her office, and she told me how fo find my friend. There was no sign-in procedure, and I was not asked for my name. This was standard practice during the 10 or 12 times I visited. IF there was a person at the front desk, that person never looked up at me or greeted me. However, there was always a bowl of candy on the counter!

When I arrived at my friend’s room, I was surprised to see her eating potato chips. When I asked her where she got them, she told me she had missed lunch due to the transfer, and had asked if she could have something to eat. A staff member brought her a bag of potato chips!

My friend arrived in a hospital gown, and that was not changed for the first three days she was there, nor was the bedding changed, even though the sheets were stained. When I complained, they got my friend dressed and out of bed for the first time in three days, put her in a chair, changed the sheets, then put her back into bed and called it “physical therapy.”

A large number of service providers, who arrived at my friend’s door, had to ask,”What is wrong with you?” They explained that they were contract providers, did not work for Crescent, and frequently didn’t receive any records about the patient, and just had ti “figure it out.” One
occupational therapist asked my friend if she was finished with her chemotherapy treatments. My friend was not receiving chemotherapy! Many days therapists didn’t show up at all.

My friend was often in pain and receiving morphine every four hours. Frequently, the pain medication was forgotten unless my friend called the nurse. It was not uncommon for the nurse to take his/her time about getting the pain medication delivered. Sometimes my friend didn’t get the medication for 6.5 hours, instead of the prescribed 4 hours!

After just two days at Crescent, I realized my friend was regressing. At SMH, she was able to take short walks, using a walker, but very soon, she started to experience extreme weakness and couldn’t walk at all. I began to worry that she could die at this facility. I tried to get her transferred to a better rehab. facility, but none of the few good rehabs had any beds available.

My friend was begging me to get her out of Crescent, and she wanted to go home, but Crescent didn’t think she was ready to be discharged. Trying to arrange a discharge at Crescent was not easy. I was told I should get my own wheelchair and walker, because it would take them weeks to get these items. I don’t know why, but a few days later they provided a wheelchair and a walker. I arranged for home health care, and after a week at home, my friend is much happier and healthier.

Please, please don’t ever send anyone who needs healthcare to this facility. I cannot think of one kind word to say about this place. Below are pictures of the peeling paint in the hallway. They posted a sign that said to excuse their appearance while they were undergoing renovations, but in the almost two weeks I was visiting, I saw no workers that were fixing the walls or anything else.

The staff are very knowledgeable and helpful people. They take care of the residents well. It is very clean and welcoming. Dayanna is very pleasant she is always smiling. The receptionist Kat always greets visitors with a welcoming presence. Stephanie and Sheakeia from admissions are amazing at the facility. I would recommend this facility because they provide excellent support to residents..

Do not go or bring a loved one here! ( TRUST ME )! My Cardiologist recommended me going to a rehab. I received a new heart valve. I was very weak and my breathing wasn’t good.My caseworker found this place and one in Lakewood Ranch. This one was on the way home for my husband. We thought that was perfect. It wasn't> it was a nightmare from the start! Read those one star reviews and BELIEVE THEM! I love how they respond to the one star reviews, but yet don’t do anything about it! I was put in a diaper! This facility doesn’t have a PureWick system like the hospital. I was given water pills and was too weak to get up. It was so humiliating. I had to place another diaper inside my diaper so I wouldn’t wet the bed. Which did happen twice.The first time was a lesson in diapers >the second time I had dropped the call button.I had to wait until someone came and checked on me 2 hours later. I finally got some strength back and used the bedside toilet. I got stuck and asked for a wider one. I didn’t get one until the day before I left! I told nine people that worked there!!!!!! The lack of communication was so evident ! I never got a shower! I was there for a week. They kept saying there were certain days you took one. They moved the whole entire second unit floor to unit four. ( without notice) I was one of the last ones moved. It was at nine at night! The next day asked about the shower, she told me to talk to the nurse. That never happened. I left on my own reconnaissance. The food was a mystery. I was told three days after I was there about a sheet you had to fill out to order something other then the special. I never got what I ordered anyway. One night for dinner we got water. The girl said they were saving money! I ordered a turkey sandwich and I received a fish patty . I told the girl and she finally brought me a sandwich. ! No mayo or mustard. It was 3 pieces of cheap white bread with a few slices of mystery meat. I went to bed hungry! I can only imagine how many patients aren’t eating and laying in urine right now! It’s a horrible facility and the State needs to know how they are treating their patients!

My mom has been at this nursing home since January and the improvements that have been made recently, can be felt in the atmosphere. The staff seem happier and much more willing to engage. Great job!
