This facility is older and could use some uodates/upgrades but the staff took care of my mother while she was there. The nurses are very pleasant and the Patient Coordinator Chasity is so so sweet. She could relax and calm my mother just by taking the time to talk and visit with her. It's one of the better facilities around. So choose this one if your family needs a temporary home.

Came from hospital to here without my agreement was expecting to be in
Plant City.No one has done Anything to do for rehab. Only at 2 meals, send eggs, sausage, milk etc back every day and every day I get same meal. Being lactose intolerance, acid reflux etc this is perfect meal. Had dinner sitting from approx 5pm until next morning at 8 am on table.
Woke up hearing girls (working here) laughing in hallway, my whole backside was exposed to hallway with door open. They had been in several times yet could not pull the curtain over.
Had been without my CPAP machine working for 5 days not my ice machine to keep my leg from swelling after knee replacement.
Leg stabilizer is never in right spot and boot is not put on my foot for broken ankle when I have to get up to go to bathroom.
In constant pain asked why no X-rays are done to make sure everything is ok, no response.

I was a patient here twice for rehab after surgery. I thought it was bad the first time...it's horrific now. Elderly people with impaired mental function wander the halls aimlessly in wheelchairs. Otherwise they are left in their beds up to 24 hours a day. You almost never see staff and they don't like to see your call light. Staff are insolent, lazy and unmotivated. It was my worst nightmare and my daughter took me out after just 5 days. Not once did anyone from Physical Therapy work with me. DON'T PUT YOUR LOVED ONES THERE.
P.S.: They are, however, very diligent in taking their smoke breaks, lunch breaks, break breaks, etc.

When my client first went in to this rehab facility I was not a fan. Only because I am not used to a facility, I am her private caregiver at home and have always done our routine a certain way. When I was told that I could no longer assist it really hurt me because for almost 7 years I have been taking care of her and there's never been an issue. Now I understand that it is only because it's their job and if something was to happen to her it would be a liability. I'm not gonna lie there are some people that I feel shouldn't work here however there are some really good CNAs,(Annette& Jessica) really good nurses (Jen) great physical therapists (Brittney & josiahs) that should never go unnoticed for the great care they provide. Kendell the unit manager, is very helpful!when my client needed a softer mattress to prevent any sores she was so quick and responsive to us. I am very thankful for her and her staff for all the kindness and support they have shown us❤️ Chasity the patient needs specialist is the sweetest person in the whole building, she is your go to person, anytime I needed something for my client (clean sheets & pillow cases, extra pillows and blankets,smaller diapers etc)she would always bring it! She is very personable and actually cares for the patients!

I would like to thank Mrs. Arleny, and Mrs. Nilza (RN), Mrs.Lourdes (CNA), The rest of the Rn, CNA that I don't have the names, including the staff, and the maintenance department at the Central Park location that they work and help my wife during the 55 days my wife was interned. God bless them for the special job that they are doing.

The facility is old and needs a lot of updates and upgrades. The exterior and the interior are not in top pleasant conditions. I hope that the board of directors know what they need to repair and clean to make the facility more attractive to bring more revenues.

I just want to say my stay here has been a good experience. My nurses, Annette, Kayla, & Jenn help me so much. As well as Chasity, she has helped me so much and is so sweet. She will get all the answers I need or find out what questions i may have. Overall I love being here.

My aunt was admitted here a few days ago. Some staff could use attitude changes but I want to shout out Bree from the business office. She told us that she was new, but took the time to explain things and was patient with me and my aunt. Much appreciated !
